Harvard University Cross Registration Form Help

The following is a detailed listing of what information is required on the cross registration petition form.

Note: This website is intended to provide cross registration information as well as a means to obtain a proper cross registration petition. Completing an online petition form does not automatically register a student into a course.

Name
Enter your last name and first name and middle initial in the spaces provided

Email Address
Enter your complete email address in the space provided. Example: john@harvard.edu

Phone Number
Enter your complete phone number, including area code, in the space provided

Degree/Program
Enter your degree program in the space provided. If you are a special student in FAS please enter "Special Student".

Society/Advisor
HMS/HSDM students should enter their Society name. Other students should enter their advisor name, if applicable.

Harvard ID#
Enter your eight-digit Harvard ID number in the space provided.If you do not have a Harvard ID please see your HOME school Registrar.

Semester/Year
Indicates the term for which you wish to enroll.

HOST School
Check the school to which you wish to enroll.

Course
All courses that you intend to study for credit or audit must be entered on the cross registration petition.
Each course that you enter on the petition form must be accompanied by its correct information:
 

Course Code/ Number - enter the course code/number or the abbreviated course name (For example, Economics 2010b is the abbreviated course name).

4-Digit FAS Catalog Number - for FAS courses only, enter the four digit catalog number which appears in the Courses of Instruction after the long course title (for example, the catalog number Economics 2010b is 8659).

Course Title - enter the course title for the course (For example, Economics Theory is the full course title).

Graduate or Undergraduate - Choose whether you are taking this course for Graduate or Undergraduate credit.

Grading Option (ordinal/pass-fail/sat-unsat/audit) - choose as appropriate.

Meeting Day/Time - enter the day(s) of the week and times that the course meets, if applicable.

Number of Credits/Units - enter the number of the course credits you expect to receive (please see your HOME school's rules regarding credit coversion).

Instructor's Signature -obtain the instructor's signature for each course listed on the petition.

Student's Signature -sign completed petition form before submitting at HOST school.

HOME School's Signature -obtain the signature or stamp of the home school's Registrar. Note that some schools require a HOME School Signature prior to getting the Instructor's signature.

HOST School's Signature -submit the completed cross registration petition form at the HOST Schools Registrar's Office for signature.

Advisor's Signature (if required) -obtain your advisor's signature if required by your HOME school.
